My mom has been at Sapphire for 7 years and she is now at the end of life and I am so grateful that she is under the 4th floor care at Sapphire. The team is amazing and go above and beyond everyday from the Deanna the tech to the nurses Fredrickka and Olga!! They are true heroes and god bless them!!

The social worker, Crystal, was informative and helpful. The guard at the reception area was polite and courtesy to me. He was also very helpful with me when I used the kiosk. The keyboard is too small especially not appropriate for seniors with eyes problems.

All the staffs in Sapphire are very caring, professional and helpful. The place is very clean. Ian & David from Nursing and Bridgett from Admissions were so helpful and very kind! I highly recommend this facility for your loved ones!!!

Because the family members only learned here that this nursing home often arbitrarily changed the time of key patient examinations without the consent of the patients and their families, delaying the best treatment time for the patients and causing irreversible effects on the patients. The staff or management of the nursing home did not follow the doctor's orders and nursing standards, resulting in the elderly being unable to undergo routine examinations on time. The nursing home failed to fulfill its reasonable duty of care (direct negligence in diet management, medical arrangements, etc.); the elderly suffered delayed illness, causing physical or psychological harm to the elderly. The nursing home signed a service contract or admission agreement with the elderly or their families/guardians, which included nursing standards, notification obligations, and how to arrange medical examinations, but they did not comply with the regulations. New York nursing homes have regulations that clearly stipulate the nursing home's obligation to inform residents, the nursing home's disregard for medical orders, and the unauthorized adjustment of patient examination arrangements without notifying family members, and the serious lack of compliant communication and records.

My family decided to put our mom at Sapphire and we don’t regret our decision. My mom stayed on the 6th flr, short term stay. All the staff from nurses, nurse aids, recreational staff to maintenance crew are caring, helpful & professional. They preformed their jobs with compassion. My mom felt safe and well taken care of. On the day my mom was released, there was some tearful moments. I highly recommend 6th flr short term stay at Sapphire.

Sapphire Center is the third rehab facility I have been in. On the day I had an appointment with my surgeon (he removed a tumor from my spine). I complained of pain in my clavicle. He said that he would x-ray it while checking my surgery. It was a fracture, so he said he's not sending me back to that place. That's when I came to Sapphire Center. I have stage 4 prostate cancer. When I got here, I was in very bad shape. To put me in the wheelchair, they had to use a hoyer-lift machine. Even then they had to put pillows under my leg because of my extreme pain. The last place I was in did nothing but try to stand me up and hold onto a wall-bar. When I screamed in pain, they just sat me back down in the wheelchair for hours. Despair fueled my tears.

Then when I went to Sapphire, I was treated with true care. I was carefully put in my wheelchair to go upstairs for assessment. Five people very carefully lifted me from my wheelchair, I cried out, not so much in pain, but fear. They eased my fear as they eased me back into my chair. “Don’t worry we got you,” they told me. And now instead of being in pain, I can get out of bed by myself and use the walker. My miracle at sapphire was only possible because of the people and care that I get here. Their encouragement fuels my desire to get better. I forgot to tell you, the first place I went to, the doctor was telling the nurses that I would never walk again. He was right. I would never walk again there, I would only have my miracle at Sapphire Center.

The people that continue making life better for me are Diane James, my physical therapist, Dorothy Racimo, the Director of the Rehabilitation Department who keeps My mental state stable. And Dilya Avezova, the second floor supervisor who has always gone above and beyond for everybody. My name is Martin Williams, and this is my temporary home.

Love Saphire and all the staff. Always clean, activities daily. My mom is well taken care for 5 years. All the floor nurses are always there for my mom. We're very grateful for Saphire.
