I went to this facility for Rehab following knee surgery. Initially staff was not up to par. After getting Administration involved the tables turned for the better.I received excellent physical therapy given by Michael and Voncelle. Von's kind demeanor and caring attitude really great encouragement for the patients she serves everyday, produces great outcome. Great job Von!! Michael was a real go-getter was always anxious to get you at a functional level. Sometimes a little aggressive lol, but only to my betterment. Thanks for the push to get me up and functional in a short period of time. Very much appreciated.

I would give this place negative if it would allow! Idk where to start but I wouldn’t send my worst enemy to this place. My mom had a stroke and needed rehab and we were told the rehab here was good but we will never know because we pulled our mom out after a few days! The care at this facility was non existent! The first night there was ants in her bed and she pushed the call button and no response for an hour! Finally they get her to a new room but there was roaches! It takes over an hour to get a call light answered and our mom is unable to walk so if she needed to use the bathroom she was stuck in a bed to either hold it or go on herself. It’s sad when your loved one says they are afraid to eat or drink because they don’t want to have to use the bathroom because no one is there to take them. Then the food is terrible, it would be cold by the time she received a tray and not edible. Then she could not feed herself, but they would set the tray down and leave. Not making sure she was set up to be able to even try to feed herself, twice she dumped the food all over and sat in her mess because no one came to check or help her. She was told they only had water at one point for her to drink. The housekeeping is nonexistent, toilet clogged, the list goes on and on! Then to call a nurse and get no response or even worse theyre rude and brush you off. We work in the medical field and know how care works and this is not acceptable! Please do not let your loved one’s go here, we are heartbroken over how she was treated 😭

My mom had a stroke and needed physical therapy to get her strength back. I was told their pt was great. First night there she calls and there were ants everywhere in her room and nobody would answer her call bell. We rushed up there and call bell had been loosened. Needless to say it just got worse. Over an hour response time to call bells. She has limited movement due to stroke so needed extra help which she didn’t get. She didn’t want to eat or drink because she was scared of having an accident on herself. Nurses complaining about patients in front of my family. Housekeeping very rude. I would not send my worst enemy here. We ended up after 6 days taking her home.

They got me to walk again after being in the hospital doped on so much meds but park meadows got to walk again and i am so thankful for them to help me. In my books they all are Awesome!! Would recommend them to Anyone!! Thanks Park Meadows!!!

The therapy team is amazing especially Mike and Patrick. I went to the hospital for 9 days and transfer to Park meadows Health and Rehab Center and I met those two guys. I could not walk, stand and did none on my own. It takes 1 1/2 week and I was able to stand, use the restroom by myself, and walk using a walker. After three weeks, I’m using a Cane to walk. And getting dress on my own. That! Is amazing.

This is the worse rehab facility I have ever visited hands down! My friend was just recently here. They didn't give or offer a shower or bed bath and he was there a week
Had just recently had surgery and his wound needed more attention than they we're willing to give. Wanted to leave him in urine covered clothes and just put a pad under him for next shift to take care of!! Didn't wipe his behind good and left him dirty!!
Terrible place to send anyone I would NOT recommend!! 😡😡😡😡🤬🤬🤬

I cannot begin to understand how this place is open! We cannot wait to get our loved one out of there.
The pictures posted of this place is NOT what this place resembles at all. Patients wandering the halls . Nurses and staff busy on their phones , laughing and joking around instead of taking care of business. Staff walking around with pants below their but , hats, hoodies covering part of their face Can't visit family in their room nowhere to sit. Roaches running around . Was there 3 hours a elderly cried for help no one checked on her she was still trying to get staffs attention when we left. I could go on and on etc... Something needs to be done about this place

The worst nursing home I have ever seen patients are not being taken care of at all. The facility is so dirty and no house keeping. Wounds are not being cleaned getting infected. The staff sitting around doing nothing. No supplies are in the rooms or available. This place need to be shut down now.
