If I could give this place less than even a quarter of a star,I would. My grandfather needed to go to a rehabilitation center after his hospital stay, so we had came across this place. Not even 2 days he spent in this sad excuse of an establishment and he was sent right back to the hospital in even worse condition than he had arrived. The staff was cold, did not care, short, and even sarcastic. My grandfather was non responsive and we were trying to get him to wake up because he wasn’t and their reaction to this was to shake him and flip him over until he was screaming in pain. They responded “see he’s responding”. They also had asked if he had an ear infection and he had so their response was to shove gauze in his ear until he screamed in pain. We found him slumped between the bed and his wheelchair with not a single person to be found to help him. He was alone in this room with no help and he needs around the clock care. He is a veteran, a good man. To be treated like this during his the most fragile and most needing part of his life is disgusting. I couldn’t even begin to explain how I felt when I saw him in the emergency room after his arrival from this evil place. Do not send anyone you love here, especially if they are elderly. They will most likely end up in the ER in worse condition too.Absolute shame.

This was my 1st visit to Optima Care Fountains. From the security gate to the lobby, then up to my cousins room. ALL personnel were curteous, receptive, and very pleasnt to talk too! The Facility itself is immaculate, obviously very well polished and maintained. While visiting, healthcare personnel entered the room twice to assure the patient and guests were comfortable! I would stongly recommend this facility for care of their loved ones!

I went here to visit a family member today. It was my first time at the facility. The guard at the front gate directed me where to park. When I went inside, I was greeted by the very sweet receptionist. She asked if it was my first time there and I replied, yes. She had me sign up using their camera system which took a picture of me as well as checked my temperature for Covid purposes. I also had to enter my name, phone number and the person's name that I was visiting and then I had to answer a couple of Covid related questions. She told me that the sign up process is a one time thing that will be used each time I visit. I feel that it's a great system being that it makes it safe for the patients and staff as to who is coming in and out of the facility. She then gave me the room number and directed me where to go in order to get to the room. On my way to the room I stopped at the nurse's station and spoke to one of the nurses. She was very polite and friendly. I took the elevator to the second floor and made my way to the room. The place isn't fancy, but it is accommodating. All and all, I had a very nice visit with my family member.

From the D.o.n to the cleaners, every body works in a professional manner. The Nurses in the North building are amazing, so also are the Nursing Assistant. The south building is amazing although the nurses bedside manners have rooms for improvement.

My story is far to long to share. So what I will say is this, I pray that someone in a high position at this facility reads this simple review and cares enough to look up my contact information in their records and calls me. So that I can share what I witnessed. In the case that doesnt happend, I'll leave you folks with this. If a shift supervisor is allowed to mistreat a patient than what example are they demonstrating to those they are supervising? If a patient is set to move in on Sept 20th but does not stay as a result of the mistreatment witnessed, why is it that this facility went on to submit paperwork on Sept 24th AFTER there was a phone conversation on Sept 23rd that confirmed everything would be cancelled.

love this place and i hate nursing homes but they treat my grandma so well and im happy this is where she is spending her last few years ❤️
