I would like to express my appreciation for the wonderful care my sister receives, with special recognition for the cheerful, professional, kind way the caregivers provide care even when the patients are not assigned to them. This is difficult work and they treat the residents with dignity and respect. The administrator, social worker and nursing staff are always available if I need something. My family appreciates the care you provide for our family member and all the residents at Oakhurst Health and Rehab. Thank you!

Absolutely terrible facility. It Needs to be SHUT DOWN. My father was sent there as a last resort with no other option with end stage liver failure. It took several days before he got his medicine which causes brain malfunction when not taken. There’s two showers in the whole building for 60+ patients one of the wasn’t even working. They can’t find the remote for his T.V so he hasn’t watched anything since he’s been in. They refuse to give him condiments for his food and lie and say they ran out and no one got any. Then you walk down the hall and can see others with condiments. The place smells like urine. It’s a depressing sight seeing the lack of care from the staff. They refused to call 911 for my father forcing me to have to do so then argued with me over the phone and said the “medical advice” given was to just have a bowel movement. That’s the best advice they gave and said if he took and ambulance it would be going against medical advice. I’m sorry to hear that you would risk the possibility of someone dying because you don’t have any compassion and care. My dad is supposed to be getting rehabilitated and while in there and they haven’t done anything but had him do a brain game. No physical therapy and he’s so weak that he had a fall while in there because no one is watching him. It also takes a week to get clean laundry. This place is a disgrace to what it means to care for another human being. Please please do not send your loved ones here. I promise you will regret it.

My Uncle receive excellent care from this facility. I couldn't be more pleased with the care and attention given to my beloved Uncle Bee. He would tell me how he enjoyed talking to the staff, particularly Ms. Stacy and Ms. Sabrina as they kept him laughing daily. And boy did he love to laugh. Very robust lol. You helped bring joy in his final days. Thank you!

I was apprehensive to move my mother into Oakhurst because I wasn’t sure just anybody could handle her, but I now see that she finally has a community who meets all of her needs.

Kiera and Sabrina made sure that each step of her transition into extended care was smooth and I finally feel free for the first time in my life. Stacey in the social work department tells me that every day is like the first (she has dementia) and they claim she’s happy! Until I come to visit, of course. Candice the nursing director checks on her regularly and all of the staff knew her by name (for good reason) within the first week and handled all of her individual needs with expertise and grace.

Oakhurst H&R could not have been a better investment for my family!

I had the pleasure of visiting Oakhurst Health and Rehabilitation recently, and I must say, I was thoroughly impressed with the positive transformation this facility has undergone. In years past, Oakhurst had unfortunately garnered a somewhat unfavorable reputation, but under the exceptional leadership of Sabrina, the administrator, and her dedicated team, including Candice, the Director of Nursing (DON), and Stacy in social work, I have witnessed a remarkable change that deserves recognition.

One of the most striking aspects of my visit was the genuine care and compassion that radiated from the staff. Sabrina, as the administrator, clearly sets a standard of excellence and professionalism that the entire team follows. Her leadership has fostered an environment of open communication and a strong commitment to the well-being of the residents.

Candice, as the DON, plays a crucial role in ensuring that the nursing care provided is of the highest quality. I could see her dedication to maintaining the highest standards of healthcare, and her attention to detail was truly impressive. Her leadership is undoubtedly contributing to the well-being of the residents, and I have no doubt that the quality of care at Oakhurst has significantly improved under her guidance.

Stacy, in social work, also deserves praise for her dedication to the emotional and social needs of the residents. Her empathy and commitment to helping residents and their families navigate the complexities of healthcare and rehabilitation were evident during my visit. It's heartwarming to see someone so genuinely invested in the happiness and comfort of the residents.

Oakhurst Health and Rehabilitation has undergone a remarkable transformation under the new leadership. The changes are not limited to the physical environment but extend to the quality of care, the professionalism of the staff, and the overall atmosphere of the facility. I have no doubt that Oakhurst is now a place where residents receive exceptional care and attention, and it's all thanks to the dedication and leadership of Sabrina, Candice, Stacy, and their incredible team. This facility has truly earned my trust and admiration.

My father in law was there for 4 days and went down hill significantly! The hospital got me set up with power of attorney papers before he went in and this place insists that he is can speak for himself. They refused to call 911 though in 4 days my dad went so far down hill he looked close to death. He fell off his bed that didn’t have any bars and had to have 3 nurses pick him up yet no one knows anything. He also had a nurse fall on his knee in which now we need to transport him back to ER for X-rays! Called thinking they could give us insight as he’s not fully capable and they refused! They said his belongings are still there as they were waiting for him to return, then when I asked if they could pull the record to let us know detail so we can explain to dr for an X-ray they claimed he had no file as he wasn’t a patent. I highly recommend NOT sending your loved one to this facility. The biggest issue I have is they seen him that way before I got there and they were fine with leaving him in that condition. Had I left him there unattended I doubt he would still be alive. He lost HUGHE amounts of weight and they messed his medication up as well as didn’t even work with his actual dr to get it right. He has super high bp as his condition and yet they seen him at 84/63 and did nothing but wait to see. The ambulance once I insisted they came and spoke of my power of attorney, they spoke nothing good on this place as well. They deal with more patients passing and not getting attention soon enough. This is completely unacceptable for the elders who got us where we are today! They deserve dignity and respect. He was there for rehab and this place set him further back than ever as well as two new bedsores! Thank team! Great work!! I really appreciate you bringing my father in law closer to death! This Will also be receiving as many complaints as I can put my hands on. The government needs to do better at watching these places and paying people who care for our elders and disabled them maybe this wouldn’t be a common issue for our elders who gave all they have to get us where we are today!! I would add pictures to show y’all what 4 days done but I’ll save that for a not on google review!

My Mom is a patient there and the care she is receiving goes above and beyond. The staff are all amazing and so respectful. I had to meet with Stacy and Victoria and they are both exceptional in what they do! The best!!

Oakhurst is near and dear to my families heart. We have multiple family members there, and they are well taken care of. I would like to thank Mrs. Agee for providing us with information about Oakhurst and for treating us like family every time we see her out in the community.
