First Coast has employees who care! The CNA’s work hard providing for the patients. Many have worked there over 15 years giving compassionte personal care. Deborah the activity director engages the patients with games, activities and parties. Recently she had a Valentine ball where patients dressed up, danced, and chose a King and Queen. It’s not always easy to find employees giving 100% to their job but you see it daily here. I’m a frequent visitor.

I'd like to commend Ms. Carolyn Toomer, in the business office for being dedicated to the residents of 1st Coast. When my dad passed away and I didn't know how I was going to pay for my dad's final arrangements, Ms. Boomer notified me that she had enrolled my dad in a program that would pay for the burial. That is VERY "forward t8hinking" and shows a commitment to providing care for her residents even aft
er death!! Can't tell you what a blessing that action brought to me.

My son with cerebral palsy was sent here for rehab. Would give 0 stars If it were an option. He required pureed food and to be fed. They put a regular tray in his room which he tried to eat on his own. He choked on the food causing aspiration pneumonia. Instead of sending him to the hospital they stopped feeding him altogether for several days without any plan to get him fed. He was unable to speak. I happened to be sick at the time so I was unable to visit for a couple days. Imagine my horror when I came in to see him. He was in bed completely naked with an NPO sign over his bed. Nobody knew anything about him. We immediately had him sent to the hospital where he had to be admitted for several weeks because of dehydration and aspiration pneumonia. Unfortunately he never recovered from this horrendous experience.

DO NOT STAY HERE! My mom was here for about 24 hours. She’s on very important medications that need to be taken on time. No medication was given to her at her stay, the excuse was they didn't have her medication yet. We could over hear the nurses laughing and making jokes about my mom and how she didn’t want to be there. When expressing our feelings about how my mom wasn’t comfortable staying there the “supervisor” Stacy tried to gaslight us with LIES to stay there. This place needs to be SHUT DOWN. They have the residents living in filth and roaches, my heart goes out to them they deserve so much better. My mom started getting super dizzy and they refused to call 911 so we had too and that’s how she got out of there. Please listen to me do not let your loved ones suffer here.

I was a patient for 6 weeks after a multiple break & surgery in my leg. Because of an insurance problem, I was stuck in there. The only good in there was therapists (PTs and OTs) and Beatrice the CNP. The head of the cafeteria was kind and bought me hot sauce. Someone went into room at night and stole my 22 year old teady bear and shampo. Why? Dunnoo. Meds would come 3 hours late. It was just a horrible place. The nurses would bang on the door and barge in. Horrible place . Also noteworthy was how dirty it was there. The floors were filthy as were the showers which were not in your room. It was a shared shower and you were only allowed 2 showers a week. They were filthy! One shower, someone's exploded adult diaper was still there with feeses smeared everywhere. The whole facility was filthy and poorly ran.

This experience for my family was terrible. The living conditions are inhumane and disgusting. My patient was sitting in her own feces until we were able to bring that to someone’s attention. Her bedding was horrible.. it was broke and her accident was going through the mattress . Her food only came when someone says something.The nurses took about 30 minutes to come assist. They claim they only bathe the patients twice a week. Her oxygen tank was nasty and was on the floor. She was scared to sleep because she was afraid a critter or a roach was going to crawl on her. Some health inspector needs to come inspect this asap. This is literally appalling and nasty . Something needs to be done! If you can stop your loved ones from coming here, do everything you can to do so.

The staff went above and beyond to make us feel like family. The admissions team went out of her way to make us feel special and respected. The staff takes extra time to make sure patients are treated well and taken care of.
