If you are unable to be hands on and practically sit throughout the day with your loved ones, do not send them here!! The facility appears to look nice, and it’s in a great location but don’t be fooled. The administration lacks care and compassion and it flows down through the staff. My mother fell here and required stitches, was released prior to her original discharge date-because I complained about the staff, and came home with lesions on her bottom and not-rehabilitated. The DON is very unprofessional and lacks care and compassion for families and their loved ones. The administrator appears to be racist, and there are some parts of the day in which it smells awful in there. I don’t have a pet, but I wouldn’t leave my pet there if I had too.

DO NOT SEND YOUR LOVE ONES HERE.
My Sister was sent there from one of the recommendations of a case worker.
My Sister passed there on December 2, 2024 & I am still not clear as to what happened. I was not suspicious until speaking with the head of Nurses who refers to herself as CoCo. I never met this women but she is the coldest unprofessional person I've ever encountered in a medical situation. I am reporting her to the Board of Nursing. I have also reported Fall Creek Rehab to the Texas Health & Human Svc.
After looking at the reviews I don't know why they are still in operations. I wish I read these reviews before my Sister was sent here...I sure hope this helps someone. These people do not care.

It was horrible. I asked for my pain medication and got it 3hours later. Plus my my blood pressure meds were going to be giving without taking my blood pressure. Only one good thing I could said what the administration was understood why I checked myself out. PLEASE DO NOT LET YOUR LOVE ONE GO HERE!!!!!

Have written confirmation of multiple phone calls to this place about admission, arrived and was told by the person we spoke with that there was never any communication and what was told was available is not available. They also claimed multiple times that they were not receiving faxes. Took a couple hours out of my work week in order to come down and handle. This. Could have just been honest over the phone instead of playing games.

I'm 71 and clear mind. Totally lucid. I went there for rehab therapy and was a nightmare and maybe one of my worst daysin life. I stayed wet all soaked in my urine for 12 hours. In that period of time 4 cna came and always told em if can change my diaper or bring a walker so i can go. Also i had so much pain abd asked for my meds and i was told the next shift will review my orders so i told her to at least give me Tylenol because my leg surgery was very painful and she gave me Tylenol, but nobody change my diaper. Around 10:30 a social worker came and i told her what was going on. She said they can't do nothing until physical therapist evaluate me. And said she was calling the pt manager who was there by 7 am. This lady came and order cna to change me that was wet blankets and everything, she said her therapists were in vacation but someone will be there later. He (nice male) came at 2 pm to take me to therapy and with shame i had to tell him i need stop at resroom ,he helped me and yhen came back. I stay in bike 15 minutes and back to room. He left a wheelchair bedside. Later i need go to bathroom i click the button wait,click again wait abd so on holding as much as possibly, nobody came, so got of bed and with God's help was able to go to restroom in the wheelchair. In my way out my daughter got there and saw me alone. She requested discharge because i didn't want to stay there alone without help. I stayed night they placed a comode toilet at bedside. And same thing nobody came to help me out of bed but I managed and did it 3 times. And urine was never taken away, I had to eat meals just eyes closed to not look at comode full. I was discharged at 3 pm. And nobody had cleaned the comode. Place is beautiful but care is terrible, if I am on sound mind and did this to me I can't imagine how bad older or alzheimer's patients are treated there. VERY BAD TERRIBLE PLACE

DON'T LET THE ELEGANT BUILDING AND PICTURES FOOL YOU. The staff is horrible, LACK OF COMPASSION. They are only there for their pay check, not to provide excellent medical care. Cleanliness not a factor or priority. Extremely poor sanitation techniques. Fear that my father is going to end up back in the hospital due to cross contamination. I know that the level of care at most facilities such as this one is not the best but for majority of the staff is constantly rushing a not providing a satisfactory level of care.

Don't bring your loved ones here. All patients are neglected. They served 99 cent raviolis one day. They brought dinner and the lunch tray was never picked up. Dinner looked like a happy meal from McDonalds. I bet Texas inmates eat better. A soiled diaper was left on bathroom floor and no trash can in the restroom. Was there for 4 hours and no one even came in to my dad's room to check on him. Heartbreaking!

Is there a ZERO star rating for this location? i don't even want to leave one star.
I spent a week trying to get in touch with director REYNA of which i was told my father did not qualify for insurance per the hospital instructions. that is another matter to deal with. but this facility responses are severely lacking unless there is money on the table. i guess lotto winners are fully welcome here. so unless you have money don't call this location. they can't follow up on insurance claims fast enough. lets see if they get their act together.
