Family member fell and busted her head. Cut her eye and hospitalized for stitches
Subsequently received 2 black eyes. Suspect abusive behavior by staff members. Medications found hidden in her food when refused to take. Profanity written on her bed sheets. Personal items such as clothing missing. Clinical staff did not possess skills to care for my family member. Hospitalized twice for infections. If you have loved ones with dementia I don't recommend this facility.

Medicaid doesn't give a lot of options, but The Collinwood was one of the few. My family member's health declined considerably after their move to The Collinwood.

The staff continually ignored our basic requests and it was clear from visitation that my family member's healthcare needs were being ignored and they were not even having basic hygiene taken care of.

But hey, if an emergency happens, don't worry! Their staff will be sure to NOT let anyone know and you will only find out after the fact when you receive a confused call from a hospital in the area asking questions about their care and what happened.

Their inability to caretake and manage the most basic (comparatively) of health issues like Diabetes and high blood pressure is almost impressive to an extent. SO many hospital stays that were wholly unnecessary and could've been avoided with even just a fraction of tact or care from the facility. (Yes, even the administrative staff)

My family opted for The Collinwood as opposed to hospice because we felt so much progress had been made at the prior facility. Unfortunately, we can all agree that choosing The Collinwood was the WORST decision we could've made.

Maybe your family member will have a different experience, but I would look elsewhere if their stay here isn't fully self-funded. Level of care isn't supposed to differ based on your financial situation, but boy was it obvious that low-income patients are shoved to the side and ignored for "big-ticket" patients.

We are a group of ladies from the East Plano chapter of the Islamic Circle of North America, or ICNA for short. We visit Collinwood Nursing and Rehab every second Monday of the month to plat Bingo with the residents. Your staff is amazing, so kind and helpful. The activity directors, Miriama first and now Cedric, always welcoming us and assisting us if we encounter any glitches.
We look forward to these Mondays. Hopefully we will continue to do this activity and bring a smile on the face of the residents.

Collinwood has a very engaged and caring staff. What they lack in "fancy" atmosphere, they make up for in what "really matters", providing a clean, safe caring environment for my loved one.

I want to thank the staff at Collinwood for yesterday's family care meeting. Alvina, Allysa and Simon really listened to the few concerns we had. My sister is very well cared for and the activities are so good. We are so pleased with Collinwood and the care that my sister is getting. Such a clean facility.

This is a great place for your loved one! The staff is wonderful from the time you check in and the care your loved one gets. I’m so glad we chose this place for my mom.

My grandmother was sent here a few months ago. She was an elopement risk and was able to get a wonderguard to have her freedom but not leave the facility unattended. As a family member, the staff was wonderful from the start. Simon (finances), Sean (Director of Nursing) and case worker were really responsive and helped craft my grandmothers needs. My grandmother even told me about a staff member who would bring her orange juice everyday because he knew she liked the BIG glass. The downfalls are it is a nursing home. The rooms are shared, your family member doesn't get any privacy, and risk of losing personal items despite labeling them. All in all, it was a positive experience as my grandmother transitioned into needing more care.
