It was my first time visiting a friend in Charter Senior Living of Hazel Crest and I was very impressed with what I saw. The facility is very clean, orderly with well established safety measures in place. The safe was warm and professional and made me feel welcome. I left feeling grateful that my friend is being well taken care of!

Very warm and home like atmosphere, the staff is very friendly and accommodating. Residents looked well groomed and cared for. The rooms, common areas, and washrooms were orderly and emaculate. I was very impressed with the total upkeep and appearance of the facility and residents.

My loved one has been at Charter for a week now, and everything is going great. We had some horrible experiences at other places. I have shopped around and personally spoke to nursing directors at no less than 2 dozen places all around western and South suburbs. Because our needs were at a higher level, it has been a very stressful experience. Finding Charter at Hazel Crest has been a relief. Everyone's kindness and service are felt and not just a sales pitch. The place is kept beautifully clean after the recent reno. Their security procedures are comforting. Jeana and the whole staff are amazing. Keep up the good work.

I am a healthcare provider, so I know what quality care looks like. I do not recommend Charter Senior Living of Hazel Crest because they do NOT provide quality care. My experience is too long to share in detail but in short; my mother was neglected and overly medicated to the point where I thought her disease was progressing and she did not have much time left. Before a nurse recommended that I place my mother on hospice at the facility, my mother was frequently falling while walking and out of bed. When I asked to see her medications, I found out my mother’s Seroquel was doubled at bedtime without my knowledge. I told them to reduce the dosage to what we agreed upon immediately. It was not until after I had her transferred to a different facility that I realized she was being given Lorazepam without my knowledge as well. Prior to her move she spoke in “word salad” and slept all day due to the change in medication. My mother is 89 years old. I would visit her 3 days a week after work. I start to see a difference in her care. She was always wet. She was forced to sit in urine-soaked pullups and her bed linen was always wet. I personally changed her linen every visit. Even her Broda chair reeked of urine. I had to wipe it down with antimicrobial wipes. When they moved her to a different room, the company that supplied the air mattress ordered her a new one because of the condition of the one she had. I was told she needed a new mattress because her old one was saturated with urine. I was unaware of the condition of the mattress, but I was aware of the urine-soaked clothes and linen because I washed her laundry. Although I complained, nothing was ever done about it. Every time I went to management about a concern, it was not addressed, I never received a call back with an apology or a plan of care to increase the quality of care, nor was there any accountability at all placed on the staff. In fact, I was avoided by top management and the facility charge nurse. Even after I gave my notice to move, they still did not care about how they treated my mother. My mother was left in her bed without a pullup or pants on, with a throw thrown over her legs, no sheet on her bed, her head facing the footboard. Her arms were wrapped up in her sweat shirt because she was cold. I was shocked. When she opened her eyes for me, they were blood shot red. I went to get the nurse and she claimed she was unaware that this had occurred. The Caregiver did not inform her there was a problem, if there was a problem! This is clearly neglect. The pad to prevent harm from falls were left on the floor leaving me to believe she was going to be left like that the rest of the evening and night. No one ever called me to explain this occurrence.

In the 8 months my mother resided there, the carpet was never cleaned. It was visibly soiled and smelled like urine in halls where the Residents live. The walls were scuffed and dirty, and the furniture in the common areas where the Residents sit and watch TV smell like urine. I would spray down the furniture and carpet in the hallways with Lysol and Febreze every time I visit. Many good nurses have left the facility due to the working conditions. Ninety-five percent of the time, they do not have adequate Caregiver staffing on the floor. As a masters-prepared nurse of almost 20 years, I feel obligated as a fellow nurse and Advocate for the elderly population to let the public know my experience at this facility. I absolutely do NOT recommend Charter Senior Living of Hazel Crest. The Director and Senior Nurse do not care about the Residents nor what type of staff they hire to provide safe quality care.

Our eldest sister has lived @ Charter of Hazel Crest about 4 years. The staff is amazing...very warm, loving, dedicated, professional.
The nurses are "round-the-clock" available, very responsive to the needs of the residents. Whenever there are any changes/concerns regarding our sister's care, they call us...immediately...to advise.
Her caregivers are very warm, loving...dedicated to her care and respectful of her as well!
We never schedule appointments...something we've HAD to do at her last facility. No matter when we visit, her hygiene is on point, and she seems calm and happy.
As for activities, there usually is SOMETHING GOOD going on. Whether there are refreshments in the Bistro (with or without live performers), the residents being taken out for movies (or watching movies in-house in their new movie room) and lunch/dinner, or doing group exercising, the activities appear to be well-planned, fun ventures.
The facility is very pleasantly designed. There are about 4, one-story, connected buildings, with each building representing a different type/level of care for those residing within. Rooms are available with or without showers. We were allowed to bring my sister's favorite paintings, bedding, etc., to help her feel more comfortable being away from her original home.
Our family has been pleased with our sister's overall care while at Charter of Hazel Crest. At this juncture, from the consistent, positive, good care of our sister our family has witnessed, I would definitely highly recommend Charter of Hazel Crest.

Worse experience; as far wise as employment goes. I got told to come in to do an application in person. However I got questioned about if I’d ever worked for the company. I’ve worked for the company for a short period of time in their Orland park area. I was told that I worked at the Hazel Crest job and walked out on the job which doesn’t make any sense at all. If I worked at the establishment and walked off why haven’t I got paid ? I told them I’ve never did orientation let alone training . Kenya lied and said she knew me and that I worked at the establishment and walked off. That has never happened I’m not sure who she is mistaken me for however my opinion on this location as far wise as the workers go does not sit well with me ..I’ve contacted corporate about the matter . That was so highly unprofessional
