Our father enjoyed his stay. Everything was great, food was good, service from the CNA were perfect and the place was clean. Martin was very attentive to all our needs. He was very kind, knowledgeable and caring.

Was looking for a care facility for my Mom when she required too much care for me and my family to provide. Went to Boynton Beach Rehab for an unannounced tour and was fortunate enough to have the Head Administrator, John Kelly, give my wife and I a tour. John was great, as were Mary and Martin in Admissions. The facility has very nice outdoor space and the food is above average. Jean and Ce Ce are excellent CNA's, Celeste is a great nurse and the PT/OT Dept. seems quite good. John Kelly makes himself approachable and gets involved to solve problems. I am very grateful for his help. The facility needs more CNA's and activities on weekends, but overall my Mom seems pretty satisfied.

Was there for my grandfather. Staff was very kind and attentive. Mary in admissions was very knowledgeable, kind, and compassionate. She knew what she was talking about and really put any worries away for us. My grandfather is very happy with the physical therapy and the staff.

All the Staff at Boynton Beach Rehab have been absolutely amazing; from Dr. Attella & Dr. Gorch; Dr. Attellas Nurse (Nickie); Admission (Mary-who helped me SO very much with navigating my moms health and creating the most smooth transition from the Hospital to this wonderful Rehab; she is an absolute gem; (Martin) super organized, helpful and friendly; John (the Director) is phenemenoal. To all the Physical Therapists, Occupational Therapists, Nurses, CNAs & Nursing Students have all been so incredibly friendly and helpful in my moms recovery. Boynton Beach Rehab is extremely well - kept and clean, serves healthy, tasty and balanced meals. Lastly, it provides me (as her primary care giver and POA) a sense of security and comfort knowing she is is wonderful hands, which I greatly appreciate it.

My mother passed away at Boynton Beach Rehabilitation Center on Lawrence Rd. I walked in at 855am and found her dead in her bed. Almost one week later, and still no call from anyone at this facility to give condolonces or explanation of what happened.

All I do know is that my mother was in a shared room, and there was clothing and also a bag with the name of a man on it. His TV was still on when I got there, The bed was dirty/soiled (see pic).

I do see that John Kelly the new administrator who started earlier this year left a 5 star review of this facility. All I can say is that we have used this facility several times over the past 10 years and was usually very pleased with the therapy department for a short term rehabilitation stay. Everything is so different now, so much worse that it is impossible for me to find anything good to say about this facility.

During my mother's short stay, there had been several issues which were not corrected due to lack of supervision. On four occasions the physical therapists brought my mother into therapy around 8am, before she was given her necessary medications. After the second occasion I asked for the supervisor of the department to contact me, yet that never happened. The 3rd time this happened, I contacted John Kelly the Administartor of the facility, and he went in and spoke with the therapy department and came back to let me know that it will not happen again. The very next day, it happened again. There was another occasion when my mother did not receive her antibiotics for UTI for over 48 hours! The aides at this facility did not keep her clean. So I contacted the infectious disease doctor to let him know that the facility still did not have her antibiotics after 48 hours, and he instructed them to send her back to the hospital immediately. At the hospital they decided to keep my mother for the next 5 days just so that she can get her IV antibiotics. I had reported these and several other issues to AHCA and Medicare.

Edit: As you can see, the same unprofessional generic response from senior management below which has been left on other reviews here as well. As of now still no call from John Kelly the Administrator or anyone else with answers as to why I found my mother dead in their facility at 855am, and exactly why she passed away under their care and supervision.

Edit 2: I finally received a call from someone at Boynton Beach Rehab Center, but instead of condolenses or support in our time of grieving the loss of our mother/grandmother/great grandmother, it was a yound lady demanding payment of $150, and said she owes this money even though she is no longer with us.

So I'm giving an update of our recent experience that my brother and I have had with this facility this Thanksgiving.. Our mother has to have. A guardian to leave the facility because she got confused one day. And started wandering off away from the shopping center where she was being picked up..

So my brother is the closest to the facility, as I live here in California and he called up the facility on Thanksgiving morning to let them know that our mother would be picked up by the Shuttle service, and that the service would be taking her to his house on the other end.

And that my brother would be receiving her right off the shuttle service. My brother called four or five times to get someone and could not reach them and call back again and finally did and let them know. They seem to be constantly understaffed.

What happened next was ludicrous because, the person in charge said that they could not release her, without a guardian and my brother told them very specifically, to remind them that he had full authority as to where she goes and what can be done with her, and that she would be picked up by the service and on the other end that HE Himself would be receiving her physically from the Shuttle and putting her “Physically” on the shuttle service back.
They acknowledged this and said everything was fine and as my brother waited for our mother and 24 guests were waiting over 90 minutes for our mother to show up but to no avail.
So my brother called back to the facility and the idiots there said that they never released her because she did not have a guardian. Why do they have Poor Management and Communication?
My brother called the Administrator of the facility, whom he knows very well and told him in a voice message that while the Administrator was enjoying his Thanksgiving with his family, we surely were not because our mother did not arrive as arranged. Furthermore, on my end, I tried to call my mother and the front desk multiple times yesterday, December 1, 2024, at least 5 times on her direct number, and lo and behold, the phone would not even ring. It would pause for 30 to 60 seconds and then just simply disconnect. I did this five times for each number and I called other numbers, even my brother in Florida, to assure myself that my phone was working properly.
This gets back to the problem that we had in the very beginning; that there is something wrong with the phone system. There are a lot of nice people working there, but they also have a very poor response time, or perhaps it is the phone system itself and I attempt to call my mother directly so I do not bother the front desk. But this is getting very old very quickly and has been persisting for almost two years.
The Administrator has yet to call my brother back yet and address this issue, which apparently he does not seem to care about. If it wasn't for the fact that our mother has made friends there and is happy with the care that she receives, we are not moving her. But we. Really would like to have this addressed as soon as possible and a permanent remedy in place.
We are paying customers and are not getting, Proper service and this needs to be addressed as soon as possible..
