I want to start by thanking all the staff at Boulevard for the care and compassion each provided to my mother.

My mother was a long term resident at Boulevard for about two and a half years. After two hip replacement, two strokes, many falls and other medical issues, home health care was just not enough anymore. As much as my mother did not want to leave her home, my brother and I had no choice but to seek more care for my mother. She lived up the road from Boulevard and we knew of the place as her husband was a resident there fifteen years prior.

My mother was not an easy resident as her mind was sharp, but her health issues continued to take a major toll on her body and mobility. I have seen first hand the compassion and help that the staff provided to and for my mother. Each time I would visit my mother at all times of the week, the place was extremely clean and welcoming. My mothers room was always well kept and many times I would see the staff helping and caring for my mother. Paulette was the Unit Manager on the wing that my mother was staying and she always would provide my brother and I with updates and the status of my mothers condition. She returned phone calls and I have to add spoke with my brother and I with suggestions to help aide us with my mothers conditions.

I met many CNA's that cared for my mother and all knew her by name and I always seen them care for my mother with compassion. I had the opportunity to talk with the Administrator named Tim and the Director of Nursing, Tanisha. Every time I would visit they were always out and about and interacting with not only the families, but the residents as well. I have seen them stop and talk with residents like they were life long friends. They new their names and situations and it always made me smile to see the care and compassion they have provided to all. They truly care and it shows. It shows with most of the staff I saw every time I would visit. It is truly refreshing these days to witness a collective group of people care for our loved ones as we would.

My mother passed away two weeks ago at the age of 84. I wanted to take a moment to stop and say thank you with my heart to all the staff at Boulevard. To watch a loved relative health fail and life slipping away is one of the hardest things a person has to do in life no matter what your faith my be. Boulevard helped me get through these past couple of difficult years. Thank you Boulevard for your caring a compassion and for providing such care.

Placing your loved one in a long term care facility is one of the hardest decisions you'll ever make. You hope that they will get the best level of care. I placed my grandfather here due to his late-stage dementia. He arrived at the beginning of September being able to walk (with his walker), talk, dress and feed himself. By the first week of October my grandfather was severely dehydrated, could no longer talk, couldn't dress or feed himself, and he had a fall in the facility. His rapid decline was very alarming and hard to witness. My husband and I took him to his neurologist who told us he needs to be hospitalized immediately. After addressing our concerns with the head of nursing about his level of care and how he needs to be placed in the ER, they stated that they treat their residents like "family" and there is no need to hospitalize him because they were doing the same tests the hospital would do. 2 days later my grandfather had to be sent to the ER because he was lethargic, not eating, and couldn't swallow. Within a week of his hospital stay he went into cardiac arrest and passed away within 3 days. Not ONCE did I receive a call from this facility to check in on my grandfather or to send their condolences after he passed away. When I arrived to pick up his belongings that were already sent to storage, I did not receive any feedback from head of staff. To say your treat your residents like "family" is completely hypocritical and it showed through your lack of care. My grandfather was neglected, period. I will say that there were a couple of very caring nurses, but overall, I'm extremely disheartened and unsatisfied with my grandfather's care at this facility. I loved him dearly and now he is no longer with us.

The Staff and Service go above and beyond to take care of the patients. My spouse has been in the facility for 3 months. He is very happy with the care. The physical therapy and Occupational Therapy is helping him get back on his feet. In addition the speech therapy is also great. The activities department does a great job of keeping the residents happy and involved.

Bastia, Sophia and nurse Michael were exceptionally great. Both therapies were great..the people and the system. Food was good. All other personnel were nice, EXCEPT the main nurses station in the south wing. They are awful. If you go up to ask a question, you stand there just waiting until they finish their personal conversations and without even saying "I'll be with you in a minute." They don't even answer the telephone when you call from outside. It happened numerous times when I was there, but also months ago when my cousin was there and his daughter tried to call in with questions. We don't bother the desk unless it's very important so it's not like we're complaining for nothing.

I Sent my mother to Boulevard Rehab center for rehab to gain a little bit more strength with mobility due to a minor stroke with no deficits. I decided to give this place a chance because my mother is also on dialysis three times out the week.In the process of 2 weeks my mother developed a deep tissue injury/pressure ulcer, hypervolemia, failure to thrive due to non-mobility) due to poor treatment and negligence, this by far, is one of the worst experience I’ve ever been through. I being the POA and care taker for my mother for 20+ years understand that there are many stressors in healthcare when it comes to the lack of resources but my mother being treated at boulevard rehab was unacceptable. I had to call 911 myself to get my mother out of there. Let me add that my mother was admitted into the hospital once again! If you care about your love one do not! I mean do not! send them there unless you want them to die. I want to add that the administration team and staff did not care about any of concerns since day one. The nurses aides are not knowledgeable about dialysis patients. It seems as if every patient gets the same diet. My mother came out looking like Oompa Loompa because they just kept giving her fluids and food she was not supposed to be eating. This whole process has been mentally traumatizing for me. When I asked the Director of nursing (Tanisha Spurlock)if we can speak on the last day my mother was there she completely refused and stated she had a meeting to attend while she was discussing my mother’s case with the social worker in front of my mother room. Mind you I’ve been raising my concerns with why she wasn’t on a Diabetic/Renal diet and fluid restriction.If I can rate this place a zero I would.NEVER AGAIN… I will definitely write a complaint to Medicare for poor service and more.

I’ve been to many facilities (4) and the one in Manhattan was the best which Manhattan is always the best. I’ve been one in Staten Island and that was the worst. I’ve been to Long Island and that was horrible so believe it or not this place is half decent. But unfortunately, there are people that don’t want to work and other people that do an excellent job. . They are a bit understaffed like anywhere else so sometimes things take a little long but all in all, I gotta be here another four weeks so I just need to take it and roll with the punches. I have Marie in the morning at seven in the morning and she’s a lifesaver. I have 11 o’clock shift the worst she doesn’t want to work. Even after speaking to a supervisor and she said she’ll tell her she still does nothing. She does the minimum which is really not good but like I say there were other ones who are very, very good. Glenora in central staffing is excellent. When she made a reservation for me with AMR Ambulance to take me to my appointment she did everything perfect! A good thing I checked on AMR because my appointment is at 1 o’clock and they want to pick me up at 1 o’clock not good so I suggest if anybody comes in, you must follow up on your own. You know the saying is when you want something done do it yourself? But after I talked to Glenora, she is also calling them to make sure this is all taken care of the right way. Like I said before she is excellent, and you can rely on her. Also , they lost some of my clothes I have hardly nothing left. Luckily Dawn, who is very nice and is really working hard on finding my stuff said she said she should find it today. I’ve been here a week so let’s pray that this happens. This is not unusual in a nursing home, so I suggest if you ever come here have them put a note in the closet that somebody in your family is going to do the laundry because otherwise it takes forever or gets lost. That part is not good. But I must tell you I’ve being in a lot of places and this one is decent snd very clean and they do listen to you when things are wrong. Most of the staff is very good and they do listen to what you say. But if You expect to get attention right away don’t even bother coming into one of these and get a private nurse at home which if I could afford I would’ve done. I’ve waited 45 minutes up to 1 1/2 hours. All the therapists are excellence and that’s what’s very important, which is the most important!
