Este lugar es lo peor para un ser humano. Mi Papa estuvo solo dos dias en el Area de Rehabilitacion y lo sacamos con un Stroke, que nosotros nos dimos cuenta cuando llegamos a verlo, el personal no se habia percatado que mi Papa tenia un stroke, personal incompetente, lugar apesta y sucio, tuvimos que nosotros presionarlos para que llamaran a una ambulancia. Este lugar solo acoge a los paciente para lugrarse, NO tienen vocación .

This facility should be shut down immediately. I do not understand why they have not lost their license and the state should make them close the doors. First of all they have a insect problems throughout the facility. The ants and other bugs are in the food and bed of the patients. My Mother was in the care of this facility and she was clearly neglected. She came into the facility walking and talking, about 145lbs. When she left she was barely 100lbs and she could not walk nor talk. She did not receive the physical therapy she was suppose to receive after residing there for 11 days. There were a list of medications given and not prescribed by her doctor. They stated they were communicating with her doctor, but her doctor had no contact with the facilities doctor or nurses. There was an IV placed and meds but had no record of what was being administered, how long or who authorized it. The nurse and facility Managers blamed each other and no one took responsibility for the negligence. Some of the facilities staff was facetious and arrogant with the way they talk to you. They were very unprofessional and the answer was always " I do not know!" Some of the staff cared and knew what is happening to the patients but fear their jobs. The doctor assigned to her from the facility not want to release my mother to get better care. He did not have any knowledge of her condition and by looking at her chart she was never seen by him in the 11 days she was at the facility. He decided to making decision regarding her care based on reports from CNA's, Nurse etc. When I came to visit my mother was saying they are trying to kill her and to get her out of there. In fact her condition worsened. My mother was here for about 11 days and we had to remove her to save her life. Please take the time and shop around before letting your love ones come to this facility.

02/11/2025 Since I have written this review this facility has provided a statement to insurance company and on her record banning her from any other facility. My mother didnt refuse treatment she refused to be treated less than human. It is customary that facility should at least provide a bath, food, medicine, and basic care which was never done for my mother. Seeing a doctors and rehabilitating her to get out of the facility was the goal. My mother at home has been recovering very well. This facility is banking on crippling your family member so they will not be able to leave only by death. Be care of this facility their good reviews are probably written by staff that don't want to lose their jobs.

I have had a great experience here. The Physical therapy has helped me. The cnas Macayla, Anissa, I loved them, Chasity was my angel. Always helped me, The nurse Rebecca is very nice and helpful. Will recommend this facility to any family.

Just wanna come on here and give it already a heads up about how horrible this place actually is if I could give negative stars I would the care here. The whole entire time has been absolutely awful. No communication whatsoever with me as a power of attorney. They have completely gone against Everything. I have told them and have not communicated with the at all. Better yet, his roommate who is blind and is unfortunately in here for life, and it has no reason to lie, even came out and told me that they have been refusing to give my grandfather Any kind of physical care and will not take him out of the bed even when he asks they say no We do not want that. And when I brought him his walker, the actual physical therapist got upset with me that I had done that No sense at all considering he should be using this to walk. I finally pulled him out of there and transportation was covered under my insurance so I definitely used it. They bring him to the wrong house Somehow over an hour away, did not have me look over the paperwork when once again I am the power of attorney would not recommend coming here and they’re also ridiculously expensive.

This place killed my mother. Along with a lot of other people. When we tried to take legal action they just filed bankruptcy and changed their name. This place along with every other facility they own is a death camp. You shouldn’t send your worst enemy to a place like this.

I have been a patient here since July 2023. The staff has saved my life multiple times. They always made sure I did the right thing. Kendell and Tonya is the best nurses here in this facility. I love my cnas Angel and Anissa who always brought me popcorn. The management team Cassie and darian is awesome as well very easy to talk to. It is going to be hard leaving here but they nursed me back to health several times and I am so thankful for my central park family and they are my family and always treated me with the upmost respect.

Horrible place..... the rooms and hallways are dirty, and staff do not care about the patients. They just sit behind the desk/counter and do not provide attention to the patients, which unfortunately are too sick and/old to complain. My husband was there unfortunately for a week last year and passed overnight and I didnt have a chance to say goodbye. Never did I see 1 nurse or volunteer come to assist him to eat, or drink or give him a bath. No one assisted him with his feeding. All of the admissions coordinator wanted was for me to submit the paperwork for Medicare....I will not refer any loved one to go to that place.
