My mother recently experienced a hip fracture and required short term inpatient rehab following her surgery. Cassandra and Don made the admissions process go smoothly. The staff were responsive to my mom‘s needs. Physical therapy and occupational therapy were highlights and she progressed very quickly with their help. Discharge was quick and easy. It is nice to have a facility like this located within our community.

Responsive and caring staff, from all levels of care givers!

My Father is a current patient after a fall over the holidays. Each member of the staff, whether they are a senior member such as, Katherine in Clinical Services, or Erna in Medical records, to Macey in Physical Therapy, and Carrie in Occupational Therapy and all the other staff who provide care every day and night, they have all been so polite, caring, responsive and respectful!

Each concern and question has been promptly addressed and resolved. As a family member not nearby, this has been a great relief to have each person assist me and my Father when asked.

I am so grateful to everyone there and encourage all family members to acknowledge the hard work they are all doing when we cannot do it ourselves!

My mother was a patient for 3 1/2 months at Grand Oaks .She was treated with respect and dignity by all of the caregivers involved. Everyone including physical therapy, nursing staff, clinical staff, administrative, secretary,Transportation and housekeeping, were very considerate and kind .
Every healthcare facility has challenges ,it’s how those challenges are met that Make the difference. We as a family Feel that Grand Oaks took every opportunity to give our mother the care she needed Any concerns that we had were addressed.
Navigating elder care is not easy but with the help of Tia and Crystal, the social workers,we were able to develop a plan and execute it even though that plan changed a few times they always supported us and our decisions tirelessly.
With Sandy, as a customer representative, we felt all of our needs were conveyed in a confidential and professional manner. She made a Very difficult 3 1/2 months Better with her up beat attitude And by being a caring individual.

I have to highly recommend if needed for a family member Grand Oaks Health and Rehabilitation at 3001 Palm Coast Pkwy SE, Palm Coast FL 32137. Is so close conveniently located to Palmcoasters east of I-95. My family member just spent 7 days there and I can say he was treated very well. Grand Oaks was bought out by Aspire and is run with care and compassion in as much as humanly possible by all those dedicated employees that are overwhelmed by the difficult tasks they have to handle 24-7. I recall some of their names: Ivonne manager, Greta nurse, assistants Michael, M. King, Sol and all the dedicated therapist occupational and physical. Also excellent Social Services Beatrice and Sandie, plus business office sweet Nicole! I am so glad that we elected out of 7 choices by Adventh Health and our Medicare/FHCP Insurance, Grand Oaks as I didn't have any long drive and we could spend more time with our relative in need. Lets keep in mind that the type of care our family requires is a very demanding and not pleasant work that requires a lot of vocation of a very special person to complete. TY to all that helped our relative in Grand Oaks.

This facility has a couple of concerned medical people especially the dietitian but the supervisor of nurses is rude, uncaring, shows no empathy towards my friend who has been there over a week and has over the last eight weeks has had two back surgeries and has been in and out of the hospital or rehab. She is having extreme pain and nausea with the strong antibiotic she's taking orally for a sepsis UTI and Instead of doing it thru an IV they keep giving it orally. She wanted to fight with me that she wasn't vomiting well at 73 I know what dry heaves and throwing up liquid and meds are. She threw in my face she had been a nurse for 32 years but she must be jaded at this point of her career because I told her you don't talk stern to her or anybody. And she disregarded my friends feelings or concerns. This place is old and not the cleanest. Friday when I was here I straightened the bedding and asked for a clean blanket since the current one had blood on it. My dear friend had asked for Kleenex and I went down the hall and found some. I've either seen young cnas eating or on their phones. What happened to nurses helping the sick

It is not comforting to leave a loved one here. You need to be really involved/present in your loved ones care, vocal and not afraid to go against ANYONE including the DON and doctors. The phone system is inadequate. Doesn't matter what time you call, the calls directed to the nursed station is NEVER answered. The staff literally just ignores the beeping phone. I DO NOT RECCOMMEND leaving a loved one here or staying here if dementia or inability to do any ADL'S unassisted is involved. BUT, physical therapy department is nothing short of amazing!

My 88-year-old dad was recently at Aspire Grand Oaks Rehab and Nursing Facility for all therapy services for overall reconditioning including strength, balance, and cognition. My concern was that he would require extra care for safety due his decreased cognitive status and I knew that may be an initial hurdle for him. My dad was admitted at night, so on his first full day we were visited by many different departments including social work, housekeeping, patient experience, therapy services and nursing. The staff were friendly and cared about patient needs. I learned quickly, all I had to do was ask and there was a department that was available to meet our needs. When I visited he was either in therapy, having a meal in the dining room, or in the day-room where a staff member monitored him at all times. There, they made sure he was safe, hydrated, gave him snacks between his meals, and provided activities. There was always activities going on simultaneously in other areas, such as bingo and live music to name a few. During the day, I could come and go and knew he was in good hands as the staff made a point to get to know him and his daily needs. Dad always had a clean diaper and that was important to me as he wasn't always aware. And when I would come by and see him, the assistant would tell me how he was doing, sometimes before I even asked. I spoke to nursing a lot, and they would graciously answer all my questions and helped in any way they could. They were problem solvers and wanted patient and family members to be happy. I also can't say enough about therapy and the encouragement they gave my dad to participate and thrive there. Patient experience and social worker ladies were always helpful and really care about the patient's stay. All the departments were always open to suggestions and followed up on any concerns I had. The names of helpful staff are too many to name individually, but I do want to acknowledge the admission staff. Transferring dad from another city to Palm Coast was seamless, as Don took care of all the details! And yes, I would bring my dad back if needed for future care, especially now that they know his specific needs and I've built a relationship with the staff that I trust.

What a relief be at Aspire at Grand Oaks! Mom, 91 yo, with dementia, had a hip repair Feb 5th and after almost 2 weeks at Flagler Health and Rehab i was getting desperate to move her. A retired nurse friend told me that Grand Oaks had changed management since November 2023 and after coming in person, i was hopeful to move mom here. It’s been 3 days and they have exceeded my expectations ! I’m so happy that mom is here, cared for gently; with much attention - a very responsive & unhurried staff & management.

Update : unfortunately, and i do mean unfortunately, their was a management change and PT staff change: they did not notify me but decided to remove PT from mother and left her without any physical training / stimulus for the month i was away ! Mother regressed: a 93 year old with dementia whom the management decided had reached her goals: when i returned, i found her having regressed and fearful to even take a single step. I signed mom out immediately and took her training in my own hands. I filed a complaint with the woman who handles such things and it’s been 3 months without a response. The nursing staff remained wonderful and the office gal was most willing to help out the best her position allowed. And there was a young woman at the dinning room who shined! I have changed my rating from 5 stars to 3. Would have been a 1 star except for these lovely humans who make up for the new PT management, their abysmal failure & the lack of accountability by new general management (& official person ( not new to this facility) handling complaints. )
