There are a select few employees here that are exceptional. Unfortunately, nobody wears any kind of badge or ID so you don't know who anyone is. I saw one badge in all the times I was there on an LPN from another area. There aren't any standard uniform colors so you can't tell between housekeeping, dietary, nursing, etc.

This place was the worst of all of my experiences in any kind of healthcare facility. I have worked in Healthcare for over 25 years. How anybody survives this place is an absolute wonder to me.

My dad was supposed to be in isolation due to the type of infection he had. Nobody wore gowns nor did they require visitors to. Many staff touched him without gloves (which is not ok, even if he's not in isolation). So, hopefully, the hand sanitizer was excellent because that was all that was between my dad in isolation and the other patients.

The food was cold. I admit that I thought my dad was exaggerating or just being picky but I witnessed many terrible meals myself. One was so bad that I asked the food services lady very kindly if it was possible to heat the food. She did not verbally respond or make eye contact. She took the food away as if I ruined her day and brought back a hot plate without saying a thing even though I told her "Thank you so much".

There was trash everywhere. I watched a heparin syringe sit in the same area for over a week. My dad had one bag of IV antibiotics hanging and the one from the day before that was empty also hanging on the pole. The trash was 1 foot away.

Since his port was not working, the IV pole kept crying about an occlusion/blockage. They ignored it and kept flushing him over and over with heparin. To avoid the alarms, the nurse? just left his antibiotic hanging and connected and just hoped that somehow gravity would magically do it's thing. I had to personally find and tell the nurse that his medicine was not being administered for her to come in. She forgot 2 times to clamp the line when she disconnected it, spilling antibiotics all over the floor. She held it over the trash and continued to let it spill out until she had the idea to clamp the tubing. She did this 2 different times, within 15 minutes. She had to get an LPN to come in and try to get things going. They ended up having to start another line. Luckily, all the antibiotics that were poured out or given so late didn't mess up his therapy. I guess we'll have to wait and see if it comes back and he goes septic again...

There were maybe 2 nurses that could answer my questions. The other nurses had no idea what was going on. I was actually convinced they were MAs and not RNs. I have no idea what their titles were but they seemed lost and confused. At least a few were nice but there were some hostile ones.

If my dad needed something, he'd put the call light on and nobody would come for a long time sometimes. The nurse's station in the 700s wing is around a corner and they can't see most of the rooms. He had to call me several times so that I could call the front desk. The front desk about 50% of the time could hear me but I couldn't hear them because their phones were having problems... the whole time my dad was there. I would have them transfer me and hopefully, the nurse would answer. I would have to do this over and over because when the nice lady up front offered to take a note to the nurse to call me, they never would. I often had to show up in person just to bring him edible food ask for pull-ups for him or, one time, ask for new bedding and change his sheets myself.

There is not enough room for me to write all of the things that we experienced at this place.

Was there for weeks as a rehab patient. The place is incredibly understaffed. I waited multiple times on the toilet with the help light on for over 25 minutes. Would have to bang on the wall to get their attention. When I saw a light on whole in the day room, I once mentioned to a tech and they said that's not my side and wouldn't go help. The kitchen wasn't very good. Would keep sending me food I was allergic to. DO NOT subject a loved one to this place.

They did the best to try to accommodate their patient's needs. Physical Therapy Dept. was outstanding. They work with the patients limitations and try to improve on whatever is in their capabilities. If family members and friends are involved in the
patients activities, their is more motivation for the patient to try harder . Being in rehab
facility is not easy at times, but it is also what you are able to put into it. God bless everyone who tried to help me do my best.

My intellectually challenged daughter was a patient on the 600 & 400 Units for several months. From the friendly receptionists to the cleanliness of the facility. The PT, OT & Speech therapy & Dietary departments worked hard to meet her needs. The dining room was a positive social experience & especially their kindness. Activities did their best to involve her as well as putting TV on & playing her CDS. The units' staff gave very good quality care. Even though she could be difficult @ times they showed kindness & patience. Because of all of you, she was able to go home recently. I thank all of you!

My husband (Carlos) was at Foothills rehab for almost 3 months for a bad diabetic ulcer. The care he received at this facility was top notch I can’t say enough good things about this place. Everyone there where extremely professional and knowledgeable in there field. Thank you all.

First time visiting Foothills nursing home. The front desk is supposed to be welcoming you and give you a feel that the facility is happy that you are visiting, well, That is the OPPOSITE of what i got. The vibe of the lady in the front desk was not nice, it felt like i was going thru an immigration officer. I understand that she is working on a sunday. But please don't put her there. It gives a bad impression about the place or maybe she needs to find another position that will be not interacting with visitors.

My husband is supposed to only rehabilitating in this place. But every time I come to visit his always laying down in that bed 24/7 soaking wet in urine and a dry poop. It upset me so badly that I have to elevate the issue and my unhappiness to the DON name Vane. She came in and saw exactly what I was complaining and promised to fix the problem. I will give this place a chance to show me that it’s not a bad place after all.

My brother is currently a patient at this facility and I am a surgical nurse. I have never in my life dealt with blatant incompetence, laziness, and neglect. I witnessed urine and feces soiled bedding just thrown on the floor, dried urine on the floor and dresser, and a patient covered in feces and witnessed the nurse walk out of his room to take a break. My brother like most of the patients at this facility is a military veteran.. When I questioned the nursing staff I was told if I don't like the facility I can have him moved, and his Dr refused to have a conversation with me and hung up on me.
